Composing a Letter Requesting a Course Substitution or Extraordinary Accommodation

  1. Clearly define your request. What accommodation/course modification are you asking for and what are your reasons for making the request?
  2. What would you like the committee to consider when making their decision? Relevant information may include:
    • Defining your disability and how it affects this particular curricular requirement,
    • Relating any prior experiences in this subject area and accommodations provided,
    • Referencing the diagnostic testing or medical evaluation that supports your request.
  3. State your intended major and degree (BS/BA) and your academic goals at the university. If you are unsure, identify academic interests and talents.
  4. How would the provision of this accommodation affect your academic goals?
  5. Summarize your request and ask if further information is needed. Provide your current address, phone number, email address and student ID #.
  6. Thank the reviewers for considering your request and sign your letter.
